Diabetic patients are seen by the practice nurse or ACP.
The diabetic clinic is held weekly. Patients are invited to attend every 6 -12 months for review.
Other routine appointments are arranged as necessary, and the care of some patients is shared between the GP and the diabetes specialist nurse at the Horton General Hospital.
It is hoped that with regular support our diabetic patients’ understanding of the condition can be increased and control improved.
